If you are seeking a signature from the State’s Attorney for a U-Visa (Form I-918), please carefully review the following guidelines.
Eligibility Requirements
To be eligible for U-Visa certification, the individual must:
- Be a victim of a qualifying crime (e.g., domestic violence, sexual assault, human trafficking, etc.)
- Possess credible and reliable information about the crime
- Have been helpful in the prosecution of the crime
Required Documents
Please supply the following information:
- A completed Form I-918 (leave the signature section blank)
- Contact information (complete address and phone number) for yourself or the requesting attorney or accredited representative
